Registering with us
We are currently accepting new patients.
We can only accept new patients who live in our practice area. This is the area bounded by Hyde Road, Wayland Road, Matthews Lane, Wilmslow Road and Swinton Grove. See our Patient Information Booklet for catchment area.
To register please bring:
- Passport. For those new to the NHS you must bring a valid visa or a letter from the consulate authorising your stay in this country.
- Proof of address for example; council tax bill, utility bill (we do not accept driving license or mobile phone bills or tenancy agreements)
- Your NHS card if you have one
- For children new to the NHS of 0-6 years old you will need to provide a record of all previous immunisations received and where they were given.
You will then need to fill in our registration form. Once you have done this you will be asked to have a health check with our Health Care Assistant. This completes the registration process. You must attend this health check before you can make an appointment with the Doctor or Nurse.

Once you have been accepted as a patient your medical records will be transferred to us and NHS England will send a new medical card to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
Please note:It is extremely important that you notify the surgery of any change to your address or telephone number.
